Without Limits Christian Center (WLCC) has put precautions in place to reduce the spread of COVID-19; however, we cannot guarantee that you or your family, including your child(ren), will not become exposed to or infected with COVID-19. Further, because of the number of individuals involved in a WLCC service and the fact that many infected individuals appear to be asymptomatic, attending this activity could increase your and your child(ren)’s risk of contracting COVID-19. By choosing to join us in person on-site you are acknowledging the contagious nature of COVID-19 and voluntarily assume the risk of being exposed to or infected by COVID-19. You also acknowledge that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of yourself and/or others, including, but not limited to, WLCC employees, contractors, volunteers, members, and participants and their families. You agree to assume all the foregoing risks, waive liability against WLCC and any other listed parties, and accept sole responsibility for any illness, injury, disability, or death to you or your family, including all claims that may arise resulting from any of these.

By reserving a seat and submitting this form, you are stating that the following is true:

1. I have not been confirmed or presumed positive for COVID-19, or if I have been confirmed or presumed positive for COVID-19, I have followed proper isolation guidelines per the CDC and have been cleared as totally recovered by my doctor.

2. I confirm that I do not have the following COVID-19 symptoms: fever, shortness of breath, cough, fatigue, chills, loss of sense of taste or smell.

3. If anyone within my company has been confirmed or presumed positive for COVID-19, I confirm I have been isolated from them.
